TELEPHONE ADVICE?
NHS Direct has staff who are able to provide advice concerning most health matters. The number is 0845 4647, and we encourage you to use this service. Web: www.nhsdirect.org.uk
We can also give you advice on the telephone, if you do not think that an appointment or visit is necessary.
The doctors have pre-bookable five minute telephone advice appointments. The
Doctor will call you at the arranged time at home at work or on your mobile phone
Please try to make sure your phone is free at the arranged time.
The duty nurse will be happy to provide general telephone advice. The receptionist
will log your request and the nurse will call you back when she is free.

HOW

CHANGE
MY DETAILS?
Our computer is linked to Gloucestershire
Health Authority for speed and accuracy of patient information. Please can you
help us to keep our records up to date by telling us when your name, address or
telephone number changes.
HOW DO I...
REGISTER WITH THE PRACTICE?
We welcome new patients who have moved into our practice area (see the map within this website).
Patients will normally be placed on the list of a doctor who has a vacancy; however,
this is simply for administrative purposes and you may see whichever doctor you
wish. For example you may see a female doctor if you have a preference despite
being on the list of a male doctor.
Please ask our receptionists about registration. You will need to complete our
registration health questionnaire and arrange for a blood pressure check and
urine test.
